Summary:
Nutrition Case Manager Under the direction of the Nutrition & Transportation Supervisor, the Nutrition Case Manager provides support to the Nutrition Program, Title IIIB services, and the Aging and Disability Resource Center through home visits, assessments, senior center assessments, senior center visits, data entry, reporting, and outreach.
The Nutrition Case Manager must adhere to ethical behavior, HIPAA, and business practices.
The Case Manager must establish and maintain positive working relationships with clients, caregivers and co-workers, and effectively utilize communication that fosters teamwork while possessing the ability to work independently to complete assessments in the field and reduce waiting lists.
Essential Duties and
Responsibilities:
Assist the nutrition program with home visit evaluations and senior center semiannual assessments.
Visit clients in their homes and assess clients for public benefits, especially Title III Older Americans Act services and other services provided by M4A.
Provide follow-up, including short-term case management, as needed or as directed.
Provide support and assistance as requested by the Nutrition & Transportation Supervisor.
Accurately and promptly document home visits, follow-ups, narratives, and units of service.
Conduct presentations at outreach events as needed.
Distribute fliers, etc.
, for other M4A programs such as SHIP, CDSME, and SenioRx.
Other duties as assigned.
Knowledge, Skills, and Abilities ( can be acquired on the job) Knowledge of community resources and aging programs.
Customer service skills to interact with clients, professional staff, and community representatives.
Ability to multi-task and manage projects.
Uphold proper phone etiquette, time management, and multi-phone systems.
Maintain HIPAA Compliance with clients, professionals and in methods of communication.
Strong problem-solving, interpersonal communication, and written skills.
Ability to work both independently and as a team player.
Ability to gather reports and compile information accurately and meet deadlines.
Knowledge of English grammar, spelling, and punctuation.
Proficiency in MS Office software, specifically Word and Excel.
Strong organizational skills, time management skills, and attention to detail.
Education /
Experience:
1.
Required Bachelor's Degree in Social Work, Psychology, Social Science, Public Health or Public Administration.
2.
Experience with long-term care, caregivers and the geriatric population preferred.
3.
Experience with public benefits and programs, and compliance preferred.
4.
Experience with case management, home visit, and client advocacy.
5.
Possess a track record of strong assessment, advocacy, communication and documentation skills.
